I usually put three asterisks just before the letter begins and after the 
letter ends, especially is there are blank lines before and after.  HTH!

My experience is that you can usually tell from the context of the surrounding 
text if a letter is beginning. if you keep the italics in tact, Braille users 
will be able to see them right away, and those using screenereaders can check 
the font and know it's italicised and thus that they are reading a letter.
