I go by the book, like the book I am currently reading "Big Girls don't Cry"
which is about an afro american growing up in the 60's and dealing with
racism and trying to make a career for herself. With certain racial words in
the book, I will rate it adult so it won't offend anyone.

Oh, yes. When I validate a book I always make my own decision. It isn't just children under 18. There are a lot of adults that are offended by and don't want to read books that have explicit sex, even if tastefully written, or that have obscenities or profanity. If I don't rate them adult, a I explain in the long synopsis that the book contains those words, so the reader or parents can make an informed choice. Violence, too.
