I think you should add the word chapter before the number even if the number is spelled out. I am not sure what the rationale for this is, but I, as a reader, would appreciate that. As I read along using a continuously reading synthetic voice it is good to know unambiguously when a new chapter comes along. A number by itself could be a page number or some other kind of number. Having once been a sighted reader I know that these things are quite clear when one is looking at a page of print, but when you have a detached robot reading to you it tends to lack complete clarity.
