I was certainly having a bad day.  I do the following.

>blank line, page break
>blank line
>page number
>blank line, blank line, blank line, , chapter name, blank line

I wonder if this has protected my chapters.  I also put the word
"chapter" before the roman numeral or number if the word "chapter"
does not appear in the printed book.
