Jill,

If you want the pagination to match the book, then you have to include all 
the blank pages and if there is an introduction or other pages with Roman 
numerals, you go to the first number page and back up to the first page of 
the Main text, numbered or not, and use the  navigation  file to set user 
pagination beginning with the earliest  page number you know  even if that 
page does not have a number just so it matches up with the book pagination. 
From the on the pages should match and the  earlier pages will have either a 
negative number or some other form of pagination ending with one page before 
your page  one.If you run into problems it may be that you missed a blank 
page or added one too many.
