I was not the original submitter of this book, but I will be more than
willing to scan and send the missing ones to you. I happen to be going to
the library this afternoon, and their website says it is available for me to
pick up so I'll grab it for now in case you get no other offers to do it for
you. If in fact you have gotten no other offers please email me at

and tell me which format you would prefer it in and your direct email
address so I can attach and send it off list.
