Remembering my seeing days I have always seen books using stars in that fashion placing them with a blank line before and after the stars. I think I would leave them that way in the proofread copy. I am a new proofreader.  I have 2 questions:

1.  For the page number, it is ok for me to begin each new page with:
      line space
      page number
      line space
      book text

2. The author uses "***" in the middle of the page to indicate a break between locations in the book. Should I use the "***"? If yes, should I use it at the beginning of the line? Should I surround it by line spaces like the author?

EXAMPLES:  from the text

  book text
 line space
       ***
   line space
  book text

EXAMPLE from proofreader:
OPTION 1:

book text
line space
***
line space
book text

OPTION 2:
book text
***
book text
