I just ran this past Carrie. Roger and Lori are correct. 

The rule of thumb is that we are mandated under copyright law to maintain the 
integrity of the print version. (Star Trek fans, think of this as the Prime 
Directive.)

The one textual exception we make is the lack of a title page. It is OK to 
insert a page with Title, Author, and Publisher. It is never OK to "edit" or 
otherwise change other parts of the book.

We can eliminate decorative elements that publishers sometimes use at the start 
of a chapter and the visual navigation clues such as headers repeating the 
title or chapter name at the top of each print page. That may seem like a 
contradiction but think of those as changes to navigation rather than content 
because visual cues become irrelevant in the new format.

Sometimes you may find that a word is hyphenated at a page break in the print 
version. It is acceptable to remove the hyphen and reconstruct the word before 
or after the page break so that the word renders correctly in a screenreader.
