I must admit I was a bit stumped at your question and had to do a bit of
research as I admit I was curious about this myself. The long and short
of it is that, if you've forgotten the 4-digit parental controls
passcode, your easiest option is to reset the phone as a new phone.
Don't restore it from a backup because the parental controls password
will be restored along with it. Here's one of the links that I found

My daughter had an IPOD, when she was under 13 years old.
I put in a pw to keep that set for the under 13 classification.
She just turned 13 and her IPOD put it onto her IPHONE.
How do I get that off?
