Are you able to make patch and bank changes in the track properties dialog in Sonar? Are you saving your work after making changes in the track properties? If so, I imagine that when you open the project, the last patches you put in, should be there. Another thing you might consider is going to the project options MIDI out page and checking the box that says patch bank search back before play starts. That way, if you have bank and patch changes that happen during the piece, the project will start with whatever patch and bank settings are present at the beginning of the piece regardless of the point at which the file was saved.
