Hi Jean,

The Canada Revenue Agency produces Braille files for a number of their income-tax related forms, e.g. http://www.cra-arc.gc.ca/E/pbg/tf/5006-r/5006-r-08e.brf. Perhaps this will give you some ideas.

I do not think that, from a Braille perspective, it makes any difference where labels are placed. i.e. I would not care if, in the print form, the label for that field was 'below' the line or not: I'd just do it as you have been with the label, a few dashes, and a space. You may have to play with this to make sure that signature lines have enough space to be signed (i.e. the dashes don't end up at the end of a line or something), but I think the form layout in print is largely irrelevant to the braille user. After all, most form layouts are done to cram as much onto a single page as possible, which is a bit of a moot point in braille.
