As you are a participant on one of our support groups, we would like to
invite you to read our new Insight Support Group Participant Guidelines if
you have not already seen them and provided the details required.
It is a brief document outlining our principles and values which we would
like participants to adhere to when attending our support groups.
On the form, we kindly ask you to provide your details as well as carefully
read the terms of the Support Group Participant Guidelines.
At the end of the form we would like you to confirm that you have read,
understood and agree to these terms prior to your attendance at our groups.
Any information provided will be stored confidentially by Fighting Blindness.
Please note that this form only needs to be completed once and we would
appreciate if you could complete the form by the 27th November 2023.
