The text of the email below would do the trick as an introduction but that
needs to be on the actual form itself, not just in the email.
Words like “allergen” when they mean “allergies” are important or the user
will not take the survey seriously.
Likewise, the misuses of “this” and other words in the text of the form
indicate poor spell-and grammar checking and will also give the responders
the impression that the survey is not serious.
Here is a possible rewrite of the note from their email:
An app for the visually impaired
We are creating an app that will verbally inform a visually impaired person
of the allergens in a food product. These ingredients include salt, nuts,
sugar, dairy , gluten, eggs, etc.
WE WOULD INVITE YOU to FILL OUT THIS FORM TO GIVE US SOME INFORMATION TO
GUIDE OUR WORK. (or something like that!)
After all that, I would not recommend giving them anyone’s email. They would
need to post the survey on the various visual impaired users lists. These
include

They should also send it to the members of VISPA.
