There is a way with an iPhone but it may need sight assistance. Print and fill
in form. Go to setting of your phone then camera then format which is at the
top. Select most compatible instead of high efficiency. Take photo of
application form. Gmail and attach the photo of form in JPEG. Wish them a happy
Christmas.

A jpeg is an image format, so it sounds like your previous method might be
the best one again. Print it off, have someone sighted fill it in, then scan
it and save as jpeg.
Frustrating though that they are not providing an accessible option.
