Apparently, they've fixed the issue on the 4S, where in a call, Voiceover goes down in volume for some of us when on loud speaker. That bug's been addressed... supposedly. Again, I've not yet tried it to see for sure. I'm updating right now as we speak, but according to the info on the update, they did mention about low audio while in a call, and that the issue had been addressed. So, we'll see. They didn't specifically say low Voiceover audio, but I can almost bet my money on that's what they're referring to.

Hi guys,
Just as a quick FYI, iOS 5.1 one was released for the iDevices. Other than Japanaese for Siri, it seems there are only some bug fixes. I don't know how this impact VoiceOver as of yet.

Additionally, the iPad3 will be shipping on March 16. You can find all of the details on the device on the Apple website, as well as a link to the keynote address for the Apple event. I am assuming we will see, of course, the same level of accessibility in the iPad 3 as we have come to know already. There are no other accessibility details that I am aware of as of yet.
