As far as I know, the dedicated sound adaptor is still needed for Orca
to work without stuttering on the Pi and I suspect this will remain
the case at least until the Pi gets another major hardware upgrade, or
at least, I wouldn't hold my breath for something as complex as Orca
defeating Wirth's Law.
As for Mate, I haven't heard of anyone trying any other desktops on
the Pi, but even on the more general Linux accessibility lists I'm on,
there's almost never any mention of desktops other than Mate and
Gnome(and as I understand it, Mate is basically Gnome Classic).
Also, as far as I know, the Raspberry VI website is up to date with
what's publically known about accessibility on the PI and the
Raspberry Pi Organization either doesn't know we exist or has little
interest in official support for accessibility.
