This very issue came up recently on the NVDA list. I have pasted the
instructions and associated emails here for your reference.
Message from Ron Canazzi:
1. Insert + N for NVDA menu.
2. Choose preferences.
3. Choose symbols and punctuation pronunciation.
4. You are placed in a list of symbols. Press the apostrophe key and you
should hear something like tick equals tick.
5. Tab once and type in what you want this symbol to say (in your case
apostrophe.) 6. Tab again to a combo box that asks you what level you want
this pronunciation at (default is all is unless you want different cursors to
speak it differently, this should suffice.) 7. Tab once to a combo box tht
says 'send actual punctuation to synthesizer.' If you want NVDa pronunciation,
the default 'never' is OK.
8. Tab to the add button and press space bar.
9. Make any other changes if you want, and then click add again and OK.
This should keep the changes permanent.

Hi,
You can find this in NVDA menu > preferences > punctuation / symbol
pronunciation. Make sure you consider whether you really want three extra
syllables every time you encounter this symbol though. I'm a big fan of
efficient reading, and that's the main reason I haven't changed mine back. you
get used to it pretty quickly. Hope this helps.

Hi everyone.
Can anyone help with an NVDA and punctuation problem? I've followed the
instructions in the user guide to get NVDA to pronounce punctuation marks such
as a full stop as a dot and an exclamation mark as an exclamation mark not as
it currently ddoes a "bang" An apostrophe as apostrophe and not "tick".
I'm not sure what I've done, but in word documents, any time I use the full
stop, comma, quotes, etc, NVDA speaks them as other punctuation marks that I've
never heard of before. As I write this message, I notice that they have
reverted to the correct pronunciations now that I'm using outlook. Could this
just be a ms word thing? Or Have I got a bigger problem to solve?
Any thoughts?

Hi Damien,
I take it you don't have one of those fancy multimedia keyboards with the Mute
button on the top left! :) Okay, so here's what you do...
* Open Notepad and paste the following line of code into it:
CreateObject("WScript.Shell").SendKeys chr(173)
* Save the file as: mute.vbs
* Create a desktop shortcut to mute.vbs
* Rename the shortcut to something helpful like:
Mute / Unmute System Volume
* Right-click on the desktop shortcut and select Properties.
* Pick a shortcut key. Must be a letter. Use M if it's available.
Your shortcut key will be Ctrl+Alt+M. Windows will add the Ctrl and Alt for
you.
* Select Minimized from the Run combo box.
* Click on OK and you're done!
Assuming everything went according to plan, whenever you press
Ctrl+Alt+M, your computer will fall silent. Press Ctrl+Alt+M again,
and sound will be restored.
