Yeah, that happened to me once: I think I took so long to fill out the
form and upload the book that the timer expired and I got a message saying
that I wasn't logged in rather than the thank you message, so I thought
the submission didn't go through and I had to submit it again, but then
someone rejected one of my submissions as a duplicate, so perhaps it went
through the first time.
