I have made such a template for my business cards in the past. I basically set 
up the template in DBT and then embossed it from there.
Note of caution:
You can make 2 columns of what you wanted printed on the Braille label.
As well, when you are calculating what you want on the Braille label, make sure 
that the number of lines on the label divides evenly into the number of lines 
on your Braille page. 
EG: 3 goes into 25 8 times leaving 1 blank line left over.
So in this scenario, you either have to add a blank line to the end of your 
template, or tell DBT to eject the page after the last line of Braille.
In this way, you can emboss multiple pages and make as many labels as you need.
