[duxuser] Question about the table of contents generator

What is the easiest and most foolproof way to use the table of contents
generator?  I have never gotten the nack of it and need to train on it.
Do the items in the table need to have a particular code associated with
them before starting the process?  Where is the cursor placed to begin
the process?  In the custom field indicating levels from 1 to 99,, can I
just erase the 99 or does something have to be in that field?  I am just
doing a single level table, chapter name and braille page number.
