To answer your last question first, perhaps you could have made a Transcriber's Note to explain the italic. However, not knowing the document, there might be other places in the document where the italic is used, which would be a problem.

We have done several sets of bylaws and documents where language is proposed to be deleted and/or replaced with other language. If I know the end user, I will discuss it with them. Some people like to have a blank line before and after the insertion or deletion.

I think that your suggestion will work well, and since you have multiple braille readers, finding a solution that is clear so all can understand is of upmost importance.
