Install SWIFT again, but this time select “Repair”.
There is an installation issue which is being addressed at this moment.

Subject: [duxuser] Word and Swift
Hi! I have DBT 11.3 SR1 and Word 2016. I have recently downloaded SWIFT and
according to the information on SWIFT I should have a braille tab on my word
toolbar which I don't have. Has anybody had the same experience and/or have any
suggestions to help me out?
