The latest version of Adobe InDesign lets you link content because it lets you create tagged PDF right from InDesign so this might help in exporting and creating Braille documents? Not sure it would help with images, but it should keep the logical document structure/reading order fairly decent?

Please, be careful when you export text from PageMaker, Quark or Indesign,
because designers frequently cut and paste little portions of text in a
variety of frames that often are not linked. It causes you export just one
or two parts of the text, not the whole content.

There are great DTP apps that work with structured documents, like Adobe
FrameMaker or Corel Ventura. You can simply write a script to convert
Ventura Tags into Duxbury codes or export text as RTF/Word and you can be
absolutely sure that all your text was exported. Unfortunatelly, just a few
designers and typesetters around the world knows the power of these
softwares.
