[duxuser] outline formatting

When preparing an outline where the entire text is in an outline format, BANA
rules say to put main entries in cell 3 with runnovers in cell 1. Subentries in
cell 5 with runnovers in cell 1, sub-subentries in 7-1, etc. The DBT English
Textbook de Bana and Word templates do not have numbered styles for 5-1, 7-1,
etc. I can obviously use spaces or something, but might these styles be added
in an upcoming DBT release?
