[duxuser] table of contents

Hello
I am using the table of contents generator for the first time. It gave me a
list of the chapters but no guide dots or page numbers. Also, if I place the
cursor right before the first chapter heading, Duxbury freezes up. If I place
the cursor right in front of the title page, the table of contents includes the
title in table. Any helpful hints would be greatly appreciated!
