I have downloaded the patch George suggested and experimented with the
Custom setting on the table of contents. I did get one to generate, not
exactly where I wanted it but not too hard to clean up. I did notice
that the guide dots are underneath the chapter headings instead of next
to the headings like I thought they should be. Is this the way it is set
up?

Hi Lucy,

Your lack of dots and page numbering is probably because you are using 
print page numbering and there are no print pages.  To fix this, you'll 
have to use the customized Table of Contents instead of the BANA which
is 
the default.  When you choose Customize, you'll be asked a number of 
questions, including whether you want to use braille or print pages.
You 
want braille.

When you generate the Table of Contents, it is placed immediately after
the 
cursor.  Placing the cursor in front of the title page will probably
cause 
the Contents to be placed in the wrong part of the document.

Let me know if you still have trouble getting the Table of Contents to
