Doesn’t surprise me sadly.
Full keyboard access has been playing havoc with VoiceOver for years! Apple
have never quite gotten a handle on it. Sad to hear if it’s gotten worse though.
Fortunately, though more so on iPad than iPhone, they’ve been adding more and
more standard keyboard commands, so this setting may not be as necessary as it
once was.
VoiceOver also has plenty of independent keyboard commands on both iPhone and
iPad.
Frustrating though.

I have just turned off Full keyboard access and now everything is behaving
correctly!
