In the syslog, I see that the driver found no DDC/EDID information and
also failed to map in the video card ROM to get the native resolution from
there.
It then tries to set the resolution to 1024x768, which I guess is not the
native resolution, and it seems it fails at setting it up properly, so you
end up with the invalid display you get.
At the end of the log I think you set the resolution yourself to 640x400?
If that's the case, managing to set the proper resolution and then
rebooting (assuming a writable install of haiku, so not running from
CD/DVD) might help. You can set the resolution using either the screen
preferences (if the glitch allows you to see what you do there) or the
screenmode command in Terminal.
