Update on my AR regarding where to put the definitions
on the A_to_D and D_to_A boundary conditions.

After a quick look at the v5.0 specification, it seems 
that the best place for this definition would be on the
bottom of pg. 104, right after this paragraph:

| SPICE, VHDL-A(MS), Verilog-A(MS) cannot process digital signals. All SPICE,
| VHDL-A(MS), Verilog-A(MS) input and output signals must be in analog format.
| Consequently, IBIS multi-lingual models using SPICE, VHDL-A(MS) or
| Verilog-A(MS) require analog-to-digital (A_to_D) and/or digital-to-analog
| (D_to_A) converters to be provided by the EDA tool. The converter
| subparameters are declared by the user, as part of the [External Model] or
| [External Circuit] syntax, with user-defined names for the ports which
| connect the converters to the analog ports of the SPICE, VHDL-A(MS), or
| Verilog-A(MS) model. The details behind these declarations are explained
| in the keyword definitions below.

        - Tx model is always driven by an ideal voltage source
          - i.e. zero output impedance
        - Rx model is always loaded by an ideal voltage source
          - i.e. infinite input impedance
        - Rx model's "A_to_D" converter is really an ideal probe which
          has an output of a floating point number
        - allowing multiple trees in one file is a good idea with
          keywords [Begin Parameter Trees] [End Parameter Trees]
          - eliminate 120 character line requirement inside these 
            keywords?
          - use virtual file name instead of tree name?
        - separate the general syntax and rules of the tree from
          specific rules which apply to AMI or *-AMS and ISS models
