There are lots of those errors in the document,
because we didn't clean up the references yet...
We are first making the editorials on the textual
content, and we will fix the references later.

I opened your doc and found a lot of "Error! Reference source not
found."inside. It seems to be some references are lost.

All,
I deleted references to PADHIER and .OPTION. I also put in a new section
"Best Practices" (page 4), which contains my suggested language for
Scaling, Global Parameter, Naming Conventions, Exponent Range, and Scale
Factors. All of these are obviously up for debate.
