I haven’t ever tried to create a group on my phone. On the MAC, under file,
you can create a new group. If I recall, You can select the new group from the
group table and add cards to it . I have also copied from my “all contacts”
list using Copy on a selected card and Paste to the name in the group list.
I am still getting used to using Return rather than interacting with the fields
when editing Contacts on the MAC. It feels more awkward than Yosemite so I
just tend to do most of my contact set up on the phone. Creating one on either
will make it viewable on all devices if you set up Icloud to do so.

Now what I do on mac in contacts after it opens if its for a new contact, press
command N and fill out the fields.
If its to edit an existing contact,after opening contacts, I find the one I
need and press command L and find the info I need to change
That’s all I’ve ever done since Yosemite.
