I don’t see the voice over option in my user/groups preference panes. I can
only choose from password or log in items and it is not in the application list
for log in items. Do I need to add Voice over in that list? Thanks.

If it's the standard login scree, you have to turn it on in User and Groups
preferences under the Login Options area. You will have to unlock the
preferences first, but then simply check the box to use VoiceOver in the login
window.
If you are speaking of the Filevault login screen, this cannot automatically
be turned on.
