Sounds like a disk fragmentation issue. I would do a fresh install from
scratch and then restore your important data from time machine or even a time
machine restore would be ok assuming you format the disk first. The key thing
is to reformat that disk and get the data restored on to it cleanly. Rotating
disks are very prone to problems like this and I’ve seen this personally.
Itunes is especially sensitive because it’s dealing with a library consisting
of large files that are probably scattered around the platters less than
optimally. reformatting and rewriting the data in one shot with a restore will
allow the data to be allocated more efficiently across the platters.
Or spend $100 or so, pull out that old 20th century technology:) and update to
an SSD which will be between 40 and 80 times faster. Will feel like a whole
new machine.
