This one I can definitely reproduce. You may remember that last week I complained that I couldn't replace my GFX APO even if I stopped the two audio services because the DLL was still open. Well, this issue is the reason why it was still open.
Once this happens, some of the audio formats also disappear from each device's Advanced tab. If I double-click one of the open devices on the Recording page, change the audio format on the Advanced Tab, and click "OK", the device closes. Once I do this for all devices (which makes sure all the record devices are closed), all my audio formats reappear.
I haven't seen the issue with the audio formats, but that's not something I would have noticed. I will look for this next time.
