Is there some reason that it is unclear that the diary entries are diary entries? Ordinarily, if the print version does not label it then it need not be labeled. That is, the text makes it clear, but if somehow it doesn't then I suppose a note could be added. As for the scene changes, again, the rule is that if there is a blank line then add the asterisks. If there is a scene change and there is no blank line then it might be jolting to you, but that just might be the effect the author intended. If there is no blank line then let it stand.

There are also other times throughout the book where the scene changes well, I'd rather say that the setting jumps from the main character's work office to the other main female character home office. Shouldn't asterisks go here also?
