I couldn't find the leaflet on the RNIB website -- must not have used the correct search terms. There is a listing of the IPA and the corresponding braille symbols in the BANA "Braille Formats" book, Rule 18, Section 4f.

In addition to purchasing the book from the American Printing House for the Blind, www.aph.org, the Braille Formats guidelines, without the examples, can be found at www.brl.org/formats. The braille symbol is followed by the print pronunciation symbol which is followed by a written description of the symbol, such as 'ethel, o-e ligature' or 'phi' or 'slashed zero, null or empty set.'

Hmm, I would use the brl.org chart with a little caution: There are a few rather obvious spelling errors ("haek" instead of "hacek", or is it the other way round?). Also I think an actual dot-number error: They give dots 1-3-4-6 for the edh symbol, the same as lower case x. (Not having the BANA book handy, but I think edh should be 1-2-4-6). I hope there aren't any more. ...

I should drop them a note. ...
