[ibis-macro] revision of repeater BIRD 156

Hi, All;

Walter and I have incorporated inputs in this week's ATM meeting into the new 
version of BIRD 156, which now includes contents of BIRD 131 and covers both 
Redriver and Retimer. Please review the attached BIRD. Your feedback is 
appreciated.

We plan to send it to Michael Mirmak to be posted and voted on in the next Open 
Forum meeting on Friday.
