I'm not sure how Bookshare's software handles bolding.  Italics come out right.

  I thought you could see size and bold in Braille.  Am I wrong?  I have a 
problem adding words to a book unless as a picture description, but will do 
what is set as a standard if I run across it.

  I like Jamie's idea because in braille you can't see the font and you
  might just think it's a flashback or a few days or hours later instead
  of a chapter.
