[duxuser] Generating a table of contents

Hello everyone. I tried generating a table of contents for the first time. I
expected to get the heading titles and their corresponding Braille page
numbers with some guide dots. I just got the titles. Any idea why this could
be happening? Where is the documentation on generating the ?table of
contents? I found a very brief blurb in the what's new section.
