Did you use "BANA" or "Customize" for the Table of Contents? I am pretty
sure that BANA expects to see print page numbers, and if there are no print
page numbers in your document, there won't be any in your Table of
Contents. I use the "Customize" radio button, and the "Braille page
numbering" button that appears in that dialogue.
All of this reminds me: I'd like to be able to save my customized version
of the Table of Contents parameters. I always customize my Table of
Contents the same way, and it's a nuisance to have to go through the
dialogue every time I want to generate one.
