Hi Paul,

Did you note this part in the Help file?

"If BANA is selected, your further choices are severely restricted, since DBT 
follows BANA recommendations for Tables of Contents. You can select what 
Heading Levels are to be included. You can decide what text you want for left, 
center and right columns. That's it!"
