[duxuser] Table of Contents

I am working on some reading passages for students using Duxbury 10.5 with
the BANA textbook template. Basically the document consists of one page
reading passages folowed by one page of exercises.  Throughout the document
I have used appropriate codes and styles.  For each new passage I used
heading1 to center the title.  Yesterday, I attempted to create a table of
contents using the Contents generator.  The contents is about a page and a
quarter long.  All items have the appropriate page number with guide dots
leading to the page number at the right margin.  However, there is a minor
problem which shows up on the second line from the bottom of the first page.
The appropriate title "Challenge: Find the Sentence!" appears on that line.
However, the guide dots and appropriate page number (42) appear on the
bottom line of the page with the appropriate page number no longer lined up
with the others.  Instead, there are three spaces after the "42" and the
braille page number 1.  The problem continues for the first three items on
the second page:  "Three In a Row", "Find the Hidden Picture!", and
A'Mazing!".  Although the guide dots for these titles appear below the
titles, the page number are at the right margin; the is no print page number
in the upper right corner.  The remainign two titles "Award Certificate" and
"Answer Key" are fine.  Does anyone have any idea about how to solve this
problem?  I tend to think it is related to the fact that the braille page
number is in the bottom right corner of the first page.  However, when I
look at the BANA formatting rules for Tables of Contents it appears there
should be a braille page number on a blank line in the lower right corner.
