Just a thought - have you tried Application Chooser?
The thing about Application Chooser is that it is not just an application
chooser.
If you can go down to the relevant application you can VO space to bring up a
menu of all the relevant open elements of an appoication, including open
windows - it is just possible that this dialogue will be visible in one of the
sub menus here.
The only other thing I can think of would be to check notification Centre to
check there is not something hovering about in there.
