Chapter heading

Text on the page

Page break

And for pages which don't contain chapter headings, it should be:
Page break
Blank line
Page number
Blank line
Text on the page
Blank line
Page break.

And to clarify, you don't need to have two page breaks in a row. I
just included the page breaks at the tops and bottoms of both pages.  You
only want one page break between pages.  More than one page break between
pages will seriously throw off the page numbering and page count. I know
this can be difficult to remember until you get used to it.  Perhaps you
might want to copy the format somewhere so that you can refer to it.  That
way you won't have to wait for someone to answer you when you need to know.

There is no absolutely "correct way", but I use a page number with blank
lines around it because page numbers are usually already in the book. That
way, you don't need to add bogus text in the book for the stripper to
remove.
