There are provisions in BANA's Braille Formats for distinctive emphasis when
it is needed. See http://www.brl.org/formats/rule03.html

There you will see: (1) Boldface. When it is necessary, represent boldface
print by the boldface indicator (456, 46) or the double boldface indicator
(456, 46, 46). The rules of usage for these indicators are the same as for
braille italic indicators. 

DBT allows for this with a translation table entitled English/American
Distinct Emphasis.

As far as I am aware, in both the "English American" (BANA) and
"English British" (BAUK) braille rules, bold, italic and underline
attributes are all represented as dots 4-6 in the output braille file.

So therefore, regardless as to whether your .dxp file contains the
beginning and ending codes for bold, italic and underline attributes,
they are all treated in the same way when the file is transcribed to a
.dxb file.

If, at some later date, you decide to open the .dxb file and
back-transcribe the file, regardless as to whether the .dxb file
contains the beginning and ending codes for bold, italic and underline
attributes, when you back-transcribe the file to a .dxp file, the
bold, italic and underline attributes are all represented by the dots
4-5-6 character, in both the "English American" and "English British"
DBT templates.

This can be very confusing, especially when the author of the document
used the three attributes interchangeably within the same piece of
text.
