David, file vault can be enabled by default at times, you might want to confirm
although I’ll consider that removed from the possible issues since you mention
you don’t have it enabled.
Remember this behavior is not unusual at all at least it seems pretty unchanged
to me. During the install process there are user changes, some things happen
in a user level and others as root. there are also different system levels
where things run. For example, you’ll sometimes see a log in screen but then a
second or 2 later you’ll get a setup process message and another install
process is kicked off separate from the user that you log in with. Typically,
I don’t wait more than 15 seconds with out speech maybe a bit more before
toggling voice over. It’s so easy to do why not it’s not like you’re dropping
to a shell and then running a third party screen reader. Maybe there’s a
better method which I’m open to but for me a toggle here and there of VO saves
time.
Also, when upgrading you get a prompt with a count down or the ability
to restart, Voice over is required to be reactivated after pressing the restart
key because again a different run level is entered with a different user.
I’m not sure what you mean about installation sounds with out voice
over. Other than the clunky sound JFW makes I have not seen anything that does
this. I’m wondering if you mean the count down timer that happens with each
install process. Try to interact and press your arrow to the right until you
reach a counter, it should say something like X minutes to install. Each time
that display changes if you have it in focus it will click, once per minute and
then when you get in to seconds once per approximate second. Other than that
once totally complete you’ll get voiceover reestablishing as normal. I can’t
speak to the auto login feature, that’s wildly insecure so I don’t enable it.
I’m probably telling you stuff that you already know, hopefully I haven’t
waisted your time. Let me know if any of this helps at all.

To be clear the login is Voiceover accessible, sort of, once you start
Voiceover.
The puzzle is why all of a sudden, people are now having silent login screens
appearing during El Kapitan OS maintenance updates even if they set their
Macs to start without the need for a login.
This can be very confusing as the first time it happened I left my Mac for
hours thinking it was simply taking an age to install a mega update, only to
find it had been sitting on a silent login screen which was preventing the
update from completing.
Once the update has finished installing Voiceover starts normally without the
need for a login screen as normal.
The annoyance is during the maintenance update process There is nothing
audible at all until you happen to try turning on Voiceover. Incidentally
neither me or my friend have ever used File Vault.

A couple of months ago afriend rang me to complain that he was suddenly
having this issue on his Mac Air. At the time I did not have the issue
either. I thought it was something he was not understanding as I was not
then getting this problem. A few weeks later then hey presto I found that
the same thing started all of a sudden happening to me also after one of
the updates.
My Mac is set to start without a need for log on as was his Air. Yet still
the Mac is pausing without speech during updates to demand a log on.
