I have not had the exact same problem. Since installing El Kapitan
though I have found that maintenance OS updates
have oddly started introducing an unspoken and unexpected log on
screen during the middle of an update process.
This did not arise when I had previous OS versions. Whilst this logon
screen is initially clunky with a button masquerading as a group i did
find that Voiceover would come on with command F5 in the normal way.
Why Apple have started introducing log on screen during update which do
not have VO speaking, even if it is the system default is a bit of a
mystery for me.

I had somebody come to look at the computer yesterday to make sure I knew what
was going on. Apparently it was updating and needed my Apple ID and password.
However, voiceover was not speaking that. I thought, during updating, voiceover
would still speak to me. At least I think that is what happened before. Has
anybody had this problem? I did push the command to get VoiceOver to speak but
nothing happened. Once updating was over it came up talking.

powering your mac. And hold it for 30 seconds. Do a disk repair and all of that
in disk utility. If that does not work I'm afraid your mac might be shot. It's
