[duxuser] dbt setup

Does Duxbury have a hard copy document (on real paper) describing firstly
DBT's default settings and, secondly some of the other options available
which might be useful to transcribers doing specific types of work.
It seems to me that such a document would cut down on the number of
questions that get repeated eachtime a new duxuser joins the list.
