Have you made sure you can't hear the volume both with and without headphones?
And turned the volume up with headphones in?

universal access again and this time I got a message that said VO changes
the gestures, did I want to continue. I answered yes but again it did

Did you somehow turn the volume all the way down. I know, simple
questions, but best to look at all alternatives. Try holding the home and
power buttons down for six seconds to do a reset on the device to see if
that fixes the issue.
