I think on one of those instances where I had to turn the volume down quickly so the phone wouldn't be bothering folks, I had simply forgotten that I had done so. Later, I was mystified when voiceover would not speak inspite of all the usual tricks. Someone who got ins imilar trouble told the the trick with the headphone.s

Ok. This also happened to me.
Plug in the headphones. Do the command to start reading the screen, (flick
up with two fingers)
While voiceover is reading, unplug the headphones, and start pressing up
volume.

   I think David's earlier suggestion of resting the phone to defaults
might be your only alternative, provided that your other sounds are
working.

Do you have it set for a triple click of the home button to toggle
VoiceOver on?  Also, can you get speech while the device is
connected to your computer?

Accidentally VO got turned off.  I went into iTunes and checked VO
under universal access and tabbed to okay and hit enter.  However
VO did not go back on. When I went back to check universal access,
VO was not checked. I did it again and again and still nothing. I
tried a 3 finger double tap in case I turned speech off as well
